New Clients

  • Most recently filed Federal and State tax returns

  • Cover page:

    • Legal name(s)

    • Date of birth

    • Social Security number

    • Mailing address

    • Phone number

    • Occupation

    • Photo of Driver License, front and back

    • Cancelled check to record routing and account number for direct deposit instructions

Returning Clients

  • Please communicate significant events, such as:

    • Change of address

    • Change of employment

    • Change of name or marital status

    • Birth of a child (name, SSN, DOB)

    • First-time rental of real estate

      • Date property was acquired​

      • Date property was first rented

      • Cost basis

      • Income statement

    • Other significant life events

New and Returning Clients

  • If you lived in more than one state during the year, please include the dates and address of each location in your cover page

  • Date and amount of any estimated tax payments made

  • Form W-2

  • Business income statement for self-employed individuals

  • Rental income statement

  • Form 1099, 1099-INT, 1099-MISC, 1099-NEC, 1099-R

  • Schedule(s) K-1

  • Form 1098 (mortgage interest)

  • Form 1098-E (student loan interest)

  • Form 1098-T (higher education expenses)

  • Form 5498 (IRA contributions)

  • Dependent care expenses (Amount, name, address, and EIN of provider)

  • Schedule H for household employees

  • All other tax documents received​​​

Massachusetts Residents

  • Rent expense (Amount, name, address of owner, and dates of residency)

  • Form MA 1099-HC
